Because this information changes regularly, it’s always a good idea to check with your health insurance for the most accurate and up-to-date information regarding your coverage. Also you may call any of our offices and we will be happy to assist you.


All payments, including co-payments and deductibles are due at the time of your visit. Please be advised that as part of our contract with insurance companies, we are legally required to collect all co-payment and deductibles from you at the time of service. If you are insured by Medicare then you are required to pay the annual deductible and/or the 20% balance of Medicare’s allowed charges, either personally or by a secondary insurance.
